TO THE PARENT: Please read this
agreement carefully. If you do not understand any provision, feel free to ask the Center Director about
it. This Agreement establishes your legal rights and responsibilities, and those of the Center, regarding
your child’s participation in the Center. Throughout this Agreement the terms “you” and
“parent” refer to the parent(s) or legal guardian(s) of the child(ren) enrolled in the Center, and the terms “Center”
and “we” refer to The Social Club, Inc. And it’s staff members. The term “school
day” means a day when the Center is open and operating.

You understand that the Center is providing care for your child(ren) so that you may have the opportunity to pursue
gainful employment or job training or that your child may participate in a social and academic setting developmentally appropriate
for his or her needs.

ENROLLMENT FEE AND PAYMENT.
A. ENROLLMENT FEE. An enrollment fee of $50.00 will be paid upon
completion of the enrollment agreement and submitted with same. This fee will be applied in full to your
first month’s tuition. If you do not attend the enrollment fee is non-refundable.
B. TUITION. Before the first
day of each billing month, you will pay the Center a monthly tuition fee of:

following month. If you enroll your child in the Center during the middle of a month, you will pay, on
or before the first day your child attends the Center, a portion of the monthly tuition fee pro-rated on a daily basis for
the period remaining in the month.
PAYMENT FOR ADDITIONAL CARE DAYS WILL BE PAID PRIOR TO ENTERING THE CENTER ON THAT CARE DAY.
3.
METHOD OF PAYMENT. Payment may be made by cash, check or money order. However,
if any payment by check is returned unpaid, you will owe a service charge of TWENTY-NINE DOLLARS ($29.00) in addition
to other amounts due, and thereafter you must pay by cash or money order. Payment must be dropped
off to the Center or mailed in to The Social Club, Inc., 2318 OLD WESTMINSTER PIKE, FINKSBURG, MARYLAND 21048 PRIOR TO THE
FIRST OF THE MONTH and the Center will not be responsible for any payment lost, stolen or mislaid before delivery to the Center
Office.
4. SUSPENSION
AND TERMINATION FOR LATE PAYMENT. If the Center has not received any monthly tuition payment by the FIRST
(1st) day after payment is due, the Center will refuse to admit your child to the Center until you pay the amount
due. If the Center has not received your payment by the SIXTH (6th) day after payment is due,
your child’s enrollment in the Center will be automatically terminated and his/her space will be offered to another
child. If applicable, the school authorities will be notified.
5. LATE
PICK UP PENALTIES. If your child is picked up after the scheduled closing time, you will owe a late fee
of SEVEN DOLLARS AND FIFTY CENTS ($7.50) PER CHILD FOR EACH FIFTEEN MINUTE INTERVAL OR FRACTION THEREOF BEGINNING AT THE SCHEDULED
PICK UP TIME. This late pick up fee is due immediately in cash to the employee on duty at the time of pick
up. Failure to follow through with this will warrant immediate termination of your child’s care.
6. CHANGES IN TUITION. You understand the monthly tuition rate is subject to change
and you agree that you will pay the new rate after the Center Office gives you ONE (1) MONTH WRITTEN NOTICE OF SUCH CHANGE.
This notice will be in the form of an informational flier placed in your child’s information folder one month
prior to the date the increase will take effect. Copies of these fliers
will be posted for the parent’s located at the entrance of the Center.
7.
ABSENCES. You are responsible for paying the full monthly tuition for each month your child is enrolled in the Center,
even if your child is absent due to illness, vacation, or other cause during the month. You must notify
the Center a week in advance of your child being absent due to vacation.
8.
HOLIDAY SCHEDULE AND SNOW CLOSINGS. The program follows the Carroll County Public School closings. When
schools are closed all clients can use The Child’s Nurturing Center in Finksburg. Registration is
confirmed by the Director. Families attending The Child’s Nurturing Center,
Inc. need to keep in mind the following holidays:

If Christmas or New Years days fall on a Saturday, we will be closed the Friday before, if they fall on a Sunday, we
will be closed the following Monday.

CLOSED AT 3PM ON THE FOLLOWING DAY:

SNOW POLICY:
ALL ANNOUNCEMENTS OF CENTER CLOSINGS ARE TO BE HEARD ON WTTR RADIO (1470AM) IN CARROLL COUNTY.
WE WILL ALSO POST CHILD’S NURTURING CENTER MODIFICATIONS ON THE WEBSITE www.childsnurturingcenter.com .
IF EITHER CENTER IS OPENED IN THE MORNING AND MUST CLOSE AFTER CHILDREN HAVE ARRIVED, THE CENTER WILL NOTIFY THE FAMILIES
AT WORK SO THEY MAY COME AND PICK UP THEIR CHILDREN.
9.
WITHDRAWAL BY PARENT OR CHANGING PROGRAMS. You must give the Center
Office at
least TWO (2) WEEKS WRITTEN NOTIFICATION before withdrawing your child from the
Center or
changing programs. If you withdraw your child in the middle of the month, the Center will refund
a portion of the monthly tuition fee pro-rated on a daily basis for the portion of the month remaining
after withdrawal.
10.
TERMINATION BY CENTER.
A. IMMEDIATE. The Center may terminate your child’s enrollment
in the Center, effective immediately, if any of the following conditions arise:
1. In the judgment of the Center
Director, the child’s behavior threatens the
physical or mental health of other children in the Center
2.
Tuition or application fees are not paid within ONE DAY after
payment
is due.
3. In the judgment of the Center Director, the parent is unable or unwilling to
support the program’s philosophy.
If
enrollment is terminated due to the child’s behavior, you will receive a refund of the monthly fee pro-rated on a daily
basis for the period remaining in the month. If enrollment is terminated immediately for any other cause,
the Center will refund the appropriate portion of the monthly fee.
B. TWO WEEKS NOTICE. The Center may terminate
your child’s enrollment upon TWO (2) WEEKS WRITTEN NOTICE TO YOU if any of the following conditions arise:
1. Any of the conditions listed
above under provision (A) assuming the Center has not exercised its right to terminate enrollment immediately.
2. In the judgment of the Center Director the Center’s
program does not meet the developmental or special needs of your child.
3. You fail to provide the necessary items such as lunch,
personal items, etc.
4. You fail to abide by any other terms of the Enrollment
Agreement.
If enrollment is terminated
upon TWO (2) WEEKS WRITTEN NOTICE, you will receive a refund of the monthly fee pro-rated on a daily basis for the period
remaining in the month.
11. MISCELLANEOUS CHARGES. If any miscellaneous
charges due the Center or any of its staff (for late pick up, returned check, field trips, etc.) remain unpaid at the time
of termination or withdrawal such amounts will be due and payable by the child’s last day of attendance.
NOTE: If you fail to mark either space in item
#12 and #13 below, we will presume you intended to grant full permission for your child to participate in any of the mentioned
activities or events.
12. FIELD TRIP PARTICIPATION. ALL FIELD TRIPS ARE
MANDATORY UNLESS SPECIFICALLY STATED. THERE IS NO CARE PROVIDED AT THE CENTER DURING FIELD TRIP HOURS.
The Centers regular program includes neighborhood walks, in-house field trips for preschool, off ground activities
for school age which involve transportation of children in buses used by the public school system. A separate
permission slip will be sent out to the families for the Center to receive your approval on each individual bus trip.

13. HEARING AND VISION SCREENING.
The local health department
will visit the center and supply a vision and hearing screening when they have the staff to do so during the summer months.

14. CERTIFICATION THAT
ALL INFORMATION IS CORRECT. The following attachments are a part of this Agreement. You
certify that you have accurately completed all the forms listed below, and that you have read and agree to abide by all the
provisions of the Agreement. You agree to notify the Center if there is any change in the information you
have supplied on the forms listed below.

2. HEALTH INVENTORY (MSDE
1215) INCLUDING PROOF OF LEAD SCREENING FOR CHILDREN UNDER 6 YEARS OLD.
3. MEDICATION ORDER (IF

15.
The following outline specifies our discipline policy and the procedural guidelines we follow when a problem arises:
The Child’s Nurturing Center, Inc. never
uses corporal punishment in any facility. Our actions for correcting behavior include only sound proven principles of psychological
and emotional reinforcement, primarily redirection of behavior.
PROCEDURES FOR PROBLEM MANAGEMENT
1. Director alerts parents to the problem behavior.
2. Director will record behavior.
3. Director will consult with the Executive Director or
Owner to develop a plan of action for behavior.
4.
Director will talk with the child’s parents, review plan and ask for suggestions.
IF PROBLEM PERSISTS
1. Director will notify Executive Director
or Owner and let them know that the problem still exists.
2.
Director, Executive Director and parents will meet and discuss the problem together and agree upon the best way to
deal with it. Criteria for the child’s continued participation in the program will be decided.
3.
If the behavior does not subside, the Director will terminate the child from the program.
THE FOLLOWING BEHAVIOR(S) WILL RESULT IN YOUR CHILD’S IMMEDIATE TERMINATION FROM THE PROGRAM:
1. Endangering the physical safety of
staff or other children.
2. Endangering him/herself.
3.
Intentional destruction of property.
4. Parental decision to not be involved in this process.
16. SEVERABILITY.
If any term of this Agreement is declared invalid or unenforceable it will be severed
and all other terms will remain
effective and they will be construed as though the invalid term did not exist.
17. WAIVER. If the center fails to
require that you comply with any term of this Agreement the Center will not be deemed to have waived its right to demand compliance
and the center may later require that you comply with such terms after notifying you that it will require compliance.
