Sensory Activities Permission Slip

This permission slip is designed to allow your child to do sensory related activities. 

These activities will provide the opportunity for your child to explore using touch, smell, sound, sight and taste.

Possible food items to be used could include:  pudding, whipped cream, cooked pasta noodles, Jell-O and crushed Oreo cookies.  Whatever the food item will be for the activity the staff will specify prior to the day of the event.
